Amber, Case Manager

Colleagues describe Amber as compassionate and helpful, always willing to go the extra mile. As a case manager, she loves connecting with in patients and their families. “The folks we work with often need to be connected to resources in the community,” she explained. “I really enjoy meeting them where they’re at and helping them feel supported and understood.”

To accomplish this goal, Amber or one of her colleagues connects with each patient prior to their discharge from St. Luke’s Hospital. Discharge planning can include anything from community resources to home health care or placement in a rehab facility. It is the case manager’s role to ensure these plans are in place and to troubleshoot any issues that arise.

Amber enjoys working at St. Luke’s and sees how the atmosphere positively affects patients. “I appreciate the culture here. Even during a pandemic, I have always felt supported by my team, which makes it easier to remain patient-focused.”
