Pre-appointment information and worksheets for Miller Creek Patients

Dr. James Donovan and the team at St. Luke’s Miller Creek Medical Clinic look forward to starting this weight loss journey with you. During active weight loss, plan to meet with Dr. Donovan once per month. After you reach your goal weight, plan to meet once every 3 months for 1 to 2 years.

Before your initial appointment with Dr. Donovan, complete the checklist below.

  1. Start a food and drink journal.
    Write down everything you eat and drink for at least 2 weeks before your first appointment.
  2. Read A Complete Guide to Weight Loss.
    This book by Dr. James Donovan can be checked out from St. Luke’s Miller Creek Medical Clinic or purchased online.
  3. Take the Complete Guide to Weight Loss quiz.
    After reading A Complete Guide to Weight Loss, complete the quiz to recap what you’ve learned.
  4. Schedule your first appointment.
    Call St. Luke’s Miller Creek Medical Clinic at 218.249.4600 or request an appointment online. Say you’d like to schedule your first appointment with Dr. Donovan for St. Luke’s Medical Weight Loss Program.
  5. Go to your first lab appointment.
    This lab appointment will be scheduled when you call to schedule your first appointment with Dr. Donovan. It will be at least a few days before your first appointment. This will include a fasting lab, where you will need to fast for 8 hours before. The scheduling staff will provide more information when you make this appointment.
  6. Fill out the Medical Weight Loss Program Medical History form.
    Download this form and complete it. Bring this to your first appointment with Dr. Donovan.
