Melissa, Heart Failure Nurse Navigator

“I meet patients where they’re at and support their journey,” said St. Luke’s Heart Failure Nurse Navigator Melissa Seibert, RN. “We work together to set goals and I help them get connected to the resources they need. I also help address any barriers they may come up against.”

Melissa works at St. Luke's Regional Heart &Vascular Center, but Nurse Navigators are an asset to any clinic or program. They help to ensure patients get support and guidance with their complex path and decrease re-admission rates. They do this by working with each patient, one on one, to develop action plans. These plans provide clear instructions, so patients know exactly what they need to do to get better.

Melissa loves her job because helping patients in this way is extremely rewarding. “The patients are the best part of my job,” she said. “I know I’m making a difference in their lives.”

She also appreciates the people she works with. “My coworkers are wonderful,” she said. “Everyone works so well together, striving to solve problems and address anything that would prevent us from delivering the best care to our patients.”
