Personal comfort care
Hospice Duluth® is St. Luke's Medicare-certified home hospice service for patients
with limited life expectancy. Hospice services are usually provided when
life is expected to be less than six months, but it's never too early
to start the conversation about hospice services. Hospice staff helps
patients and families focus on improving the quality of their remaining
days by providing medical, emotional, social and spiritual support.
Hospice care can enable patients to remain where they are most comfortable–in
their home, or care may be provided at a skilled nursing facility, assisted
living facility or group home. If more intensive medical care is needed,
Hospice Duluth® offers home-like, inpatient care at St. Luke's Hospital.
Under the supervision of our program's medical director, professional
caregivers work with family members and the patient's physician to
develop a personalized plan of care and support. We are expertly staffed
by registered nurses, home health aides, social workers, counselors, dietitians,
chaplains, volunteers, and physical, occupational and speech therapists.
St. Luke's Hospice Duluth® services are available within 40 miles of
St. Luke's Hospital. Patients are accepted into the program after a physician referral, assessment
by hospice staff and identification of a primary caregiver.
