Care Where You Need it Most

St. Luke's Home Care and Hospice Duluth® now offer home telemonitoring to patients.  The system lets physicians and nurses closely monitor patients who no longer require hospitalization, but would benefit from close observation after discharge.  Patients easily transmit their vital signs and related information from home and are also seen by a St. Luke's Home Care nurse at regular intervals.

This new technology offers peace of mind on those days when a home care visit is not scheduled.  It also identifies small changes in health status before they become serious, and helps individuals stay independent and in the comfort of their homes.

Easy to Use

A monitoring unit is placed in your house to measure blood pressure, heart rate, oxygen levels and weight every day.

  • At a set time each day, a voice prompt provides instructions for the monitoring process.
  • The information is collected and the results are sent through your telephone line to a registered nurse at St. Luke's Home Care.
  • The nurse evaluates the information and initiates follow up, if needed.

Common Questions and Answers About Home Telemonitoring

I don't know how to use a computer.  Can I still have home telemonitoring in my home?
You don't need a computer.  The home care nurse will hook up the unit to your telephone and program it for you.  The nurse will make sure you understand how to use the equipment and is available for any questions you might have.

How do I get a home telemonitoring unit in my home?
Talk with your doctor and/or call St. Luke's Home Care at 218-249-6111.

How much does it cost?
If you qualify for skilled home health care, this service is included as part of your home health benefit. We will help you determine it you meet eligibility guidelines. St. Luke’s Home Care is now able to provide home telemonitoring to persons interested in continuing their daily monitoring after they are no longer eligible for Medicare covered home health services. There is a daily fee for the service which would be on a self-pay basis. Home telemonitoring is also available to persons who have never received skilled home health care services. This service would be on a private pay basis.  

Home telemonitoring is funded, in part, by a grant from St. Luke's Foundation.
