Heart Failure Clinic

Heart failure occurs when the heart is unable to pump enough blood to meet the body's needs. This condition may be caused by injury to the heart muscle from a heart attack, problems with the heart valves, high blood pressure, toxic exposures, viruses, for from unknown causes. Individuals with heart failure often experience shortness of breath, congestion in the lungs, decreased exercise tolerance, swelling of the legs or abdomen, and increased fatigue. They tend to be readmitted to the hospital frequently.

St. Luke's Heart Failure Clinic provides coordinated, comprehensive care for heart failure patients. Our objectives are to help patients live full and productive lives, reduce unnecessary emergency rooms visits and reduce hosptial readmissions. This is accomplished by:

  • Educating and supprting the patient and family
  • Managing symptoms through lifestyle changes and medical therapy
  • Assisting the patient and family with prescription and medication management
  • Mnitoring the response to therapy during regular office visits and with telemonitoring technology.
